Initial Visit

On your first Visit to see Dr. Pommerville Please bring the following

1. Your B.C. Health Care Card or other proof of insurance.
2. List of Allergies
3. List of current medications. Name, Dosage, Frequency
4. Records from your family Doctor or other Specialists that you might have at home.
5. Complete the Patient info form and History form.

Payment:

For non-insured Urological procedures we accept Visa, cheque as well as cash.

Location:

Corner of Hillside Ave. and Shelbourne St. across from Hillside Mall.

Parking:

There is surface parking (limited to 30 minutes) and ample underground paid parking off of Hillside Ave. and Shelbourne St.

Pommerville Urology has affiliations with UBC, UVIC and The Island Medical Program.
Please be advised that at the hospital and in the office Dr Pommerville may be accompanied by medical students and/or residents in training.
